There are 6 steps to become qualified to participate in Bourne High School's Athletics.
1) Submit your Physical Exam document 2) Acknowledge Reading the Eligibility Information 3) Submit the Eligibility Form 4) Submit the Medical History and Authorization 5) Pay Your Activity Fee Please read and complete all of the guidelines and forms that are included. Your submittal signifies your consent. Step 1: Athletes must have a current physical exam on file in the Nurse's Office. Physicals are eligible for 13 months from exact date of last physical. Once you have your child's physical completed by a doctor, please submit the paperwork to the school nurse. |
